1、胰岛移植临床应用进展,南京大学医学院附属鼓楼医院普外科 江 春 平,胰岛移植,胰岛分离,胰岛纯化,新型免抑剂,胰岛移植,纠正糖代谢异常,防止微血管病变,胰岛移植的适应症,胰岛移 植,同种异体,自体移植,外科源性糖尿病,型 糖尿病,免疫耐受网(ITN)建议,18-65岁病史5年胰岛素强化治疗仍不能控制血糖低血糖发作伴意识障碍在一年半内有因难以控制的血糖二次以上的住院史伴有糖尿病并发症,IDDM实施胰岛移植的标准,糖尿病并发 症,视力 模糊,心血管 病变,神经性 疼痛,肾功能损害,临床应用进展,1978 Sutherland等报道首例自体胰岛移植截止到2000年12月31日，据国际胰岛移植登记处(
2、ITR)2001年6月报告全球共 51 个研究中心开展了733例成人胰岛移植,临床应用进展,1990-2000年进行的自体胰岛移植140例64%(41/64)停用胰岛素的持续时间7天47%(27/57)停用胰岛素的持续时间1年71%(15/21)停用胰岛素的持续时间1年(移植的胰岛数量 300,000 IEQ)最长的自体胰岛移植后停用胰岛素达13年,临床应用进展,1990-2000年进行的同种异体岛移植治疗355例19%(52/281)停用胰岛素的持续时间7天11%(27/237)停用胰岛素的持续时间1年最长胰岛移植IDDM不用胰岛素已达6年最长胰岛移植胰腺切除后外科源性糖尿病不用胰岛素达4年
3、8个月,临床应用进展,1998-2000年间进行的胰岛移植术后1年不用胰岛素者高达80%(28/35)明显减少胰岛素用量者为14%其结果可与同期胰腺移植媲美2,2002年8月弗罗里达国际移植会议统计报道近年来胰岛移植治疗糖尿病成功率已接近80%,临床应用进展,1990-2000年全球39个中心388例IDDM同种异体胰岛移植139例胰岛-肾联合移植1年胰岛移植物功能存活率高达79%3,临床应用进展,疗效不理 想,供体的短 缺,全球胰岛移植例数徘徊于20-50例之间,+,=,胰岛分离纯化保存方法,胰岛散在整个胰腺约占其总重量的1%-2%供体胰岛组织的制备是决定移植效果关键常用的胰岛分离方法有胶原
4、酶消化法和机械分离法均不同程度地损伤胰岛组织致其活力下降，从而影响临床移植效果,Ricordi 胰岛自动分离法,是胰岛分离技术的突破能最小化对胰岛的损伤胰岛在持续消化中进行性释出可避免再受酶的作用人为干预很少明显提高有活性的胰岛产量达到最佳移植效果现已被广泛采用,胰岛纯化,Ficoll密度梯度分离法Picoll密度梯度分离法 Rose采用磺酰基氟化物抑制 胰腺内丝氨酸蛋白酶的活性能 明显提高胰岛分离产量和纯度,胰岛组织的贮存,单纯UW保存PFC/UW双层法保存胰腺较Ricordi等常规采用PFC/UW液在胰岛分离前2-8小时预处理胰腺，以增加产量Ricordi 组报道采用PFC/UW双层法保存
5、9例“边缘”供胰,胰岛移植的方法,Ricordi胰岛分离技术最小化缺血时间足够的移植胰岛数量不含激素的免疫抑制治疗 经皮经肝PV穿刺肝内移植,Edmonton方案 加拿大Edmonton Alberta 大学移岛移植组,免疫抑制治疗,赛呢哌作免疫抑制诱导雷帕霉素 0.1 mg/kg 维持血药浓度12-15ng/ml低剂量FK506 1mg bid,控制血药浓度3-6 ng/ml,Edmonton方案的临床效果,过去十年移植后1年不用胰岛素者为8%方案头7例移植后1年不用胰岛素100%19例胰岛移植后1年不用胰岛素 80%胰岛移植后2年 60%此结果被ITR称为胰岛移植史上的里程碑ITN资助全球
6、9个中心共同验证并在推广,胰岛移植的免疫保护,阻碍移植临床效果的问题是免疫排斥所致移植物功能丧失克服这一障碍和增强免疫耐受已取得一些令人瞩目的进展,胰岛移植的免疫保护,采用抗CD3单克隆抗体阻止T细胞活化抗CD154单克隆抗体封闭活化的T细胞 输注供体骨髓细胞造成嵌合诱导耐受免疫隔离微囊化胰岛,提高移植物的存活,中和细胞毒因子阻止胰岛细胞凋亡基因治疗一些药物 已酮可可碱 异搏定 Vit E Vit PP,胰岛移植的并发症,和手术操作相关并发症 门静脉出血 门静脉栓塞转氨酶的升高高胆固醇血症高血压,胰岛移植的并发症,Edmond 等报道30例54次5例发生门静脉出血2例出现门静脉栓塞46%伴不同
