2、，建立信息公开、社会多方 参与的监管制度”。医疗卫生监管体系的建立已成为医疗卫生体制改革必须解决的重 要问题。 区域卫生信息平台是医药卫生体制改革的重要支柱。利用现代卫生信息技术和区 域卫生信息平台整合医疗卫生资源，更好地为公众提供医疗卫生服务和为卫生管理部 门提供决策支持的同时，能更好地对医疗卫生各领域进行监管。基于区域卫生信息平 台为基础构建医疗卫生监管系统是医疗卫生监管体制改革建设中的一种全新模式，对 提高监管工作效率，保证监管客观、公正，增加监管透明度，加快医疗卫生监管信息 化建设，实现医疗卫生监管的科学化有着重要意义。 目的 本课题的研究以区域卫生信息平台为依托，在对医疗卫生监管各领
3、域信息 资源整合的基础上，构建医疗卫生各业务领域统一的、多方参与的、实时协同的医疗 卫生监管系统。主要研究目标包括：统一医疗卫生监管资源，构建协同高效的医疗 卫生监管应用平台；实现医疗卫生网络化实时监管；构建信息公开、社会多方参 与的监管机制。 方法 本课题采用的研究方法有文献研究法，现场调查法，系统分析法，信息资 源规划以及软件工程与数据库设计方法等方法。利用文献研究法归纳分析国内外现有 卫生监管相关系统平台发展现状，总结已有的相关经验，为课题的研究提供理论依据 和借鉴内容；利用现场调查法进行个人深入访谈，初步了解医疗卫生监管系统功能需 求和信息需求；利用系统分析和信息资源规划的方法对医疗卫
4、生监管系统进行全面分 析；利用软件工程与数据库设计方法进行系统整体框架、功能体系和数据库设计。 结果 详细分析了医疗卫生监管系统的目标，层次和地位，组织结构和工作职 1 华 中 科 技 大 学 硕 士 学 位 论 文 能，功能需求和信息需求；对系统的整体框架、功能体系、数据中心和数据交换体 系进行了设计；对医疗卫生监管资源分布 Web GIS 系统进行了设计。 结论 医疗卫生监管系统整体框架包括基础层、支撑层、数据层、应用层和访 问层五个层次以及标准规范体系与安全标准体系两大支柱；医疗卫生监管系统功能 包括医疗卫生服务监管、公共卫生监管、卫生资源监管和医疗保障监管四部分。数 据中心以监管数据分
5、布模式为依据进行设计，主要分为数据采集平台、数据交换平台、 决策支持数据平台、数据利用等部分；医疗卫生监管系统交换数据库包括空间专题 数据库和医疗卫生监管系统业务数据库两部分。其中医疗卫生监管业务数据库包括医 疗卫生服务类数据库、公共卫生类数据库、卫生资源类数据库和医疗保障类数据库四 大类数据库；对医疗卫生监管资源分布 Web GIS 逻辑架构以及界面功能进行了设 计。 关键词： 区域卫生信息平台 医疗卫生监管 监管信息系统 系统 分析 系统设计 2 华 中 科 技 大 学 硕 士 学 位 论 文 ABSTRACT Medical and health regulation is an imp
6、ortant basis for guaranting healthy development of medical and health services, and is an important form for Country to manage medical and health services. CPC Central Committee and State Council on Deepening the views of medical and health system clearly proposes to establish strict and effective s
7、ystem of medical and health regulation, to strengthen medical and health regulation, to improve the health supervision and law enforcement system, to strengthen regulation of behavior and quality of medical and health, to strengthen regulation of access and run of medical and health, to strengthen r
8、egulation of social health, to improve regulation of medical care, to strengthen drug regulation, to establish information accessible and participatory system of regulation and so on. The system of medical and health regulation has become an important problem of medical and health reform which must
9、be solved. Regional health information platform is an important pillar of medical and health reform. Using modern health information technology and regional health information platform to integrate medical and health resource, not only provide better medical and health services for public and provid
10、e better decision support for department of health management, but also regulate medical and health services in all fields better. As a new model of construction of medical and health regulation reform, establishing the system of health regulation base on regional health information platform is of g
11、reat significance to speed up the construction of medical and health regulation information, guarantee fair regulation, increase regulatory transparency, improve regulatory efficiency and achieve the scientific management of medical and health regulation. Purpose This subject research construct all
12、business unified, multi-stakeholder involvement, and real-time collaboration system of medical and health regulation, relying on regional health information platform and the integration of information resources of medical and health regulation in all areas. Main research objectives include: Unified

14、on. Method The research methods used by this subject include document retrieve, locale investigation, system analysis, information resource planning, software engineering and database design. Summarize and analyze domestic and abroad development status of system or platform of health regulation, usi
15、ng document retrieve method, and summarize the existing relevant experience to provide the theoretical basis and reference content for research projects; Adopt locale investigation to conduct depth interview, in order to preliminary understand functional requirements and information needs of system
16、of medical and health regulation; Adopt the methods of system analysis and information resource planning to comprehensively analyze the system of medical and health regulation; Adopt the methods of software engineering and database design to design the overall framework, functional system and databa

Result Exhaustively analyze the objects, level and status, organizational structure and job functions, functional requirements and information needs of system of medical and health regulation; Design the overall framework, functional system, data center and data exchange system of sys

26、硕 士 学 位 论 文 1 绪论 1.1 课题研究背景 1.1.1 项目背景 医疗卫生监管是保障医疗卫生事业健康发展的重要基础，是国家进行医疗卫生事 业管理的重要手段。近年来，医疗卫生领域的问题突显，具体表现在：医疗服务市场 混乱，医疗保障分配不公，社会公共卫生问题日益严峻，医疗与食品安全事故层出不 穷，医患关系日益紧张等等。这些问题的出现，有着复杂的原因。一方面是医疗卫生 领域的复杂性特殊性的结果，另一方面也与政府监管缺位和监管失灵有关。如果不重 视加强监管，会导致所有现行的医疗卫生问题更趋恶化。因此，医疗卫生改革必须重 视医疗卫生监管体系的建设。 2008 年卫生部成立了医疗服务监管司（简
27、称医管司），2009 年出台的中共中央、 国务院关于深化医药卫生体制改革的意见中明确指出：“建立严格有效的医药卫生 监管体制，强化医疗卫生监管，健全卫生监督执法体系，强化医疗卫生服务行为和质 量监管，加强医疗卫生机构的准入和运行监管，加强社会公共卫生的监管，完善医疗 保障监管，加强药品监管，建立信息公开、社会多方参与的监管制度等等”。这一系 列举措预示着医疗卫生监管体制的健全与完善是医疗卫生体制改革建设过程中的必 要组成和必然趋势。 从全国卫生信息化发展规划纲要 2003-2010 年开始，卫生信息化发展迅速， 全国各大地区城市陆续出台卫生信息化发展规划并进行了区域卫生信息平台以及相 关系统的
28、研究与实践，如上海市医疗信息化十一五规划、天津市卫生信息化建设 十一五规划、广东省卫生信息化发展规划（2004-2010 ）。在不断的研究与实践中， 人们越来越一致地认识到，通过区域卫生信息平台建设，利用现代医疗卫生信息技术， 对医疗卫生资源进行整合，能更好为公众提供医疗卫生服务，更好地对医疗卫生各领 域进行监管，更好的为卫生决策部门提供决策支持。随着卫生部基于健康档案的区 域卫生信息平台建设指南（试行）、基于健康档案的区域卫生信息平台建设技术解 6 华 中 科 技 大 学 硕 士 学 位 论 文 决方案（试行）以及综合卫生管理信息平台建设指南（征求意见稿）等文件的发 布，区域卫生信息化与区域
29、卫生信息平台的相关理论初见雏形并日渐成熟。在区域卫 生信息平台基础上探讨医疗卫生监管系统的构建是医疗卫生监管体制改革建设中的 一种全新尝试，为医疗卫生监管体制的完善提供了新的思路。 在上述背景下，为加快卫生监管信息化建设，实现卫生监管工作的科学化、信息 化管理，保证监管客观、公正，增加监管透明度，提高监管效率，深化卫生监管体制 改革，推进政府职能转变，加强政府对公共卫生的监管能力，满足人民群众日益增长 的卫生服务需求，基于区域卫生信息平台的医疗卫生监管系统研究已成为医疗卫生事 业和医疗卫生信息化建设向纵深发展必然需要解决的重要研究课题。 1.1.2 国内外研究概况 为提高卫生监管效率，加快卫生
30、监管信息化建设，实现卫生监管工作及相关管理 工作的信息化和区域卫生监督执法信息共享，国内外社会和学者对卫生监管相关信息 系统、网络或平台的开发应用等方面进行了相关研究。 【 】 Detmer，Don E 1 认为要提高国家健康监管水平需要加强对个人健康管理、卫 生保健服务，公共卫生和卫生研究等四个主要卫生系统的建设；Anon ，Ministerio 【 】 Saude 2 则强调科学信息在加强全国卫生系统管理的作用；而 Vanderwerf M K ， 【 】 Hartgerink A E，Doherty C 3 等人则发明了一种客户机/设备服务器系统，该系统允 许监管人员进入客户端相关数据库
31、收集卫生数据以利于监督管理的进行。 相比之下，国内有关卫生监管相关系统、网络或平台的研究则丰富复杂得多。不 少文件对卫生监督、卫生监管系统、网络或平台均有阐述。如，2002 年发布的全国 卫生信息化发展规划纲要 2003-2010 年就明确指出，依托国家卫生信息网，建成覆 盖全国各级卫生监督机构的网络系统，互连互通，实现卫生监督数据信息共享；卫生 部在 2006 年发布的卫生监督信息系统建设指导意见中也明确提出“十一五”期 间，要建成覆盖全国的卫生监督信息网络平台，建立健全卫生监督信息标准体系，完 善卫生监督信息系统业务应用软件，建立卫生监督数据信息共享交换平台，实现卫生 监督工作实时、动态和
32、科学管理，规范卫生监督执法行为，提高卫生监督工作效率； 2009 年发布的中共中央、国务院关于深化医药卫生体制改革的意见中也明确提出 7 华 中 科 技 大 学 硕 士 学 位 论 文 要建立实用共享的医药卫生信息系统、建立严格有效的医药卫生监管体制。 国内不少省市业已建成全省或全市统一的卫生监督信息管理系统、网络或平台。 如，浙江省于 2007 年 2 月启动卫生监督信息系统研发工作并采用 B/A/S （Browse Application/Sever ）架构在全国率先建成 “一网一库一系统”（“一网”即全省一级网 络支撑平台；“一库”即全省集中式卫生监督数据库；“一系统”即浙江省卫生监督信
33、 【 】 息系统），该系统具有卫生许可、日常监督、行政处罚、办公自动化四大应用模块；4-5 上海市正式建成卫生监督综合应用平台，该平台整合了卫生监督所有业务系统，可开 展卫生许可、监督检查、行政处罚、投诉举报等业务，初步搭建了上海卫生监督信息 化管理的总体框架，优化了卫生监督管理的模式和业务流程，为提高卫生监督的质量 【 】 和效率打下了的基础；6 北京市建成供北京市 18 个区县卫生监督人员统一使用的卫 生监督执法综合管理信息系统，该系统涵盖食品卫生、公共场所、生活饮用水、职业 卫生、学校卫生、传染病与消毒、医疗机构、血液管理、放射卫生、母婴保健和人类 生殖等 10 个专业领域，采用全新的执法模式，通过使用 POS 机、PD
