34、准，准备必要的检查测试工具、仪器及法律文书等执法检查准备工作，必要时，会同有关部门联合组织生产经营单位生产现场安全监督检查。涉及专业性内容的，也可以邀请相关专家参与生产经营单位生产现场安全监督检查，并听取专家的意见和建议。检查中，应当作出书面现场检查记录，由检查人员和被检查单位负责人分别签字，存档备查，并及时把执法检查记录录入全市安全生产监督管理暨重大危险源预警信息系统。要统筹兼顾，上下沟通，要优先安排每月到期必须进行复查的生产经营单位。确需对计划作重大调整的，应当按照有关规定审批和备案。（三）严格执法，健全执法检查考核制度。要依法严肃查处安全生产领域的非法违法行为，督促生产经营单位全面落实安

38、安监局关于开展“职业卫生监督执法年”活动的通知精神，结合本地实际制定“职业卫生监督执法年”活动实施方案。开发区、各街道安监站根据区安监局“职业卫生监督执法年”活动实施方案，结合年度工作，认真组织实施。（二）推进阶段（5月16日-10月31日）。结合年度执法计划和“百日”执法活动，严厉查处各类违法违规行为，对执法检查中发现的各类违法违规行为，依法给予警告、罚款、责令停止产生职业病危害的作业等行政处罚；问题严重的，要依法提请地方政府予以关闭；对查出的问题，要逐条提出整改措施、明确时限要求，确保整改到位。区安监局适时对各地“职业卫生监督执法年”开展情况进行督查和指导，确保按照方案要求，完成职业卫生监
39、督执法年活动工作任务。（三）总结阶段（11月1日-11月10日）。开发区、各街道安监站要按照本方案所列附表内容，做好本地区执法信息的汇总上报工作，并于11月10日前将活动开展情况的报告连同附表报送区安监局职业健康科。四、工作要求1、加强领导。开发区、各街道安监站要切实加强对开展“职业卫生监督执法年”活动的组织领导，明确工作目标、任务，组织配备执法人员和装备，督促执法计划实施，逐级抓好落实。2、统筹推进。开发区、各街道安监站要把开展“职业卫生监督执法年”活动与职业卫生基础建设活动、职业病危害专项治理等监管工作有机结合，进行统筹安排、统一推进。3、督查考核。区安监局将结合全年重点工作，开展“四个专

41、点“百日”行动主要针对安全隐患、职业危害相对突出的重点地区和企业。重点地区指：有亡人事故发生，或高危企业相对集中、涉及职业危害企业数量相对集中的街道、行政村。重点企业指：事故频发、隐患众多，安全生产基础管理薄弱的企业；职业危害相对突出的企业。二、“百日”行动内容（一）重点地区查街道、行政村安全生产监管责任的落实和事故隐患的排查治理工作是否到位和是否有效督促企业依法开展职业病防治工作。（二）重点企业1、查企业安全生产的主体责任是否有效落实；安全生产教育培训是否到位，安全投入是否到位、应急救援（预案及演练）是否到位、基础管理是否到位；查企业在生产经营活动中是否存在非法、违法行为。2、查企业职业病防
42、治工作是否依法开展（职业病危害项目是否及时、如实申报；是否请有资质单位进行检测；是否如实告知员工职业病危害真实情况；是否对接触职业病危害因素的员工建立健全健康监护档案）。三、“百日”行动组织本次“百日”行动由区安监局统一协调，全区安监执法队伍联合行动。具体分三个阶段：（一）组织计划阶段（4月15日4月25日）各科室（大队）、各安监站要广泛宣传本次“百日”行动的目的意义，依靠基层安监组织摸排本次专项行动的重点，并于4月22日前汇总上报本辖区“百日”行动的重点地区和企业，4月25日前区监察大队汇总并制定全区“百日”行动计划。（二）具体实施阶段（4月26日7月15日）各科室（大队）、各安监站安全生产
43、监察人员按计划到各重点地区、企业开展“百日”行动。（三）总结评估阶段（7月16日7月25日）7月16日前各科室（大队）、各安监站上报“百日”行动情况总结，区安监局根据工作成效进行绩效评估，并予以通报。四、“百日”行动要求（一）加强领导。各科室（大队）、各安监站要高度重视“百日”行动工作，加强组织领导，妥善处理“百日”行动与日常执法检查的关系，并与年度执法计划有机结合，确保“百日”行动取得实效。（二）严格执法。对于在“百日”行动中发现的安全生产、职业卫生的非法、违法行为，要坚决依法查处。对于情节严重、影响恶劣的安全生产非法违法行为，要予以媒体曝光。（三）廉洁自律。所有参加“百日”行动的监察执法人

45、法组织实施危险化学品安全使用许可证颁发和管理；完成化工装置设计安全诊断工作，推进“两重点一重大”自控技术和监控系统的完备与运行；巩固和深化化工生产企业专项整治；提升危险化学品安全标准化管理水平；落实企业安全生产主体责任，提高企业安全生产水平，全力防范事故发生。三、工作措施（一）进一步推进危化领域本质安全提升1.深入推进危化品本质安全提升专项行动。继续按照专项行动方案，加强组织领导，列出进度计划，推进设计安全诊断和整改验收工作，年内全面完成未经正规设计的在役化工装置设计安全诊断工作，其中，涉及“两重点一重大”的设计安全诊断工作必须于6月底前完成。要积极指导和协调，细化落实措施，解决重点工作和普遍
46、突出问题，有序推进工作。2.强化危化品“两重点一重大”安全管理。组织对危险化学品安全法规规章和文件政策的学习研究，加大宣传贯彻组织力度，指导督促企业贯彻落实和依法生产经营。要将涉及重点监管危险化工工艺、重大危险源的企业作为监管重点，开展计划查、重点查、回头查等多种形式的监督检查，全面检查企业执行落实“两重点一重大”安全管理规定的情况。重点检查企业生产装置和设施自动化控制系统装备完善与自控改造以及自控系统投运情况，发现自控系统应改未改、改而弃用的情况应作出责令停产整改、行政处罚等处理。同时，也要检查督促企业做好大型危化品储罐安全检验检测和安全类计量器具检验检测以及防雷防静电等专项工作。重点监管危险工艺以不定期检查为主，每年覆盖检查一次；重大危险源以执法检查为主，每年覆盖检查一次。定期组织梳理与核实，全面掌握“两重点一重大”企业基本情况和安全管理动态。3.注重危化建设项目安全源头监管。要按照危化建设项目管理权限和职责分工，组织做好危化建设项目的安全审查和竣工验收工作。要认真贯彻落实国家安监总局、住建部关于进一步加强危险化学品建设项目安全设计管理
