2、Basic Requirement,CMR應使用英文填寫；CMR shall filled in English所有表格中的“*”項目必須填寫；All items marked with “*” are mandatory fields產品中含有四種金屬的廠商至少應識別一家以上冶煉商；More than one smelter shall be identifiedChecker中不允許有紅色背景項目；Item with red background in “Checker” is unacceptableCMR應保存為標準EXCEL格式，否則無法上傳；CMR shall be saved a

5、laration （宣告）,Declaration填寫要求Declaration Requirement,公司名稱Company Name,Company Name： 應填寫同富士康交易的法人全稱Full name of legal body is required 請勿填寫簡稱、縮寫Do not use abbreviation,宣告範圍Declaration Scope,Declaration Scope: 供應商應選擇適當的範圍，應包含所有交貨富士康的產品Please select the proper class which could cover all the product sh
6、ipped to Foxconn “Product”選項應在“Product List”內寫出產品編號Product list is needed if Product level is selected,唯一識別碼Unique Identifier,Company Unique Identifier: 獨特的識別碼可以填寫DUNS或者VATThe unique identifier could be DUNS or VAT VAT Number，Value Added Tax Number，指增值税发票号 DUNS: Data Universal Numbering System，经常被译为
7、“邓氏全球编码”， 就是一种商业实体或组织机构的标识符。,聯絡人及管代信息Contact Info.,Contact/Representative Info.:公司聯絡人、衝突礦產管理代表姓名、郵箱必須填寫Name and e-mail is mandated,完成日期Effective Date,Effective Date:報告生效日期此日期應晚於調查所包含的年度，應包含被調查年度的管理信息，如對2013年的調查，此日期應晚於2013年12月31日The effective date shall cover the investigated year, e.g. , The investi
