32、录243分层注水测调作业安全管理制度431 简介1.1 测调中队概况测调中队直属XXX公司，于2010年2月26日正式成立投产，为油田各采油单位提供分层注水的测调服务。中队岗位设置：经理、班组长、操作员、井口工、配置岗、驾驶员。主要设备有测井仪器车、参数仪、数控地面操作系统、生活车等。主要作业项目是水井调配。1.2 主要生产工艺和流程图生产工艺：分层注水测调作业分为四个部分，即生产前准备、接收任务、现场施工、返回驻地。生产前准备包括：日常物资的准备、设备及仪器维修保养、仪器连接检查等；接收任务包括：明确任务井位、施工设计方案、领派车单和工程报表、上井前的物资准备、班前会、测调仪器借领及安装调试
33、；现场施工包括：施工现场的布置、放喷管及仪器安装、参数采集、调配；返回驻地包括：交录取资料、交报表、还测调仪器、编写并上报测调报告等。43测调生产流程图：1.2.1 测调前生产准备生产准备内容测调生产准备工作是测调作业的重要环节，为了保证顺利完成测调任务，测调中队的相关业务人员要根据生产需要做好日常的物资准备，保证日常物资的充足，以便及时上井。主要是：1、按时保养好测调车辆、发电机、绞车及其附件、防喷装置及其举升装置和附件、滑轮组等，并做好记录；2、保养好下井测调仪器、地面监控系统，以及防毒面罩、正压呼吸器等防护器材，做好记录；3、备好防渗布、棉纱、吸油毡、放喷盘根等器材；4、备好并保养管钳、
34、扳手等工具；生产准备流程图：1.2.2 接收测调任务接收任务时需要做的工作内容接到甲方测调通知单后，主要做好以下工作：1、学习研究甲方提供的井身、井筒、配注数据和其它方面的资料信息，编制测调方案；2、详细了解井位位置和行车路线、路况及安全行车需注意的事项等，领取出车单；3、领取测调仪器、工具等物资器材，并检查完好情况；4、检查测调车辆、绞车、防喷装置、液压举升装置及其附件的完好、齐全情况；5、检查作业人员劳保护具穿戴情况；6、召开作业前会议，安排测调工作和安全环保工作。接收任务工艺流程图：1.2.3 测调现场施工工艺和流程图现场施工工艺流程： 1、井场勘察：辨识施工环境风险；与甲方采油工办理井
35、口交接手续；确定工具、车辆摆放位置。 2、现场负责人召开施工前安全环保专项会议。3、搬卸工具、仪器等到预定位置，摆放并固定好测调车辆、发电机。4、安装防喷装置、连接测调仪器、地面监控系统，并由专人对此项工序检查验收、确认。5、打开井口阀门，起动发电机，下放测调仪开始测调作业，并录取测调动态数据。6、上提测调仪直防喷管内，关停发电机。7、关闭井口阀门，释放防喷器压力，在井口铺好防渗布，拆卸放喷装置，处理污染，收集仪器、工具并装车。8、向采油工交接井场。现场施工工艺流程图：1.2.4 测井返回驻地工艺和流程图返回驻地工作：1、清洁、保养仪器、工具等，并向器材保管员验交。2、整理测调资料，编写测调报

39、可靠，工作成绩突出，员工拥护，身体健康，能团结和领导全队员工完成各项生产任务。责任技术员：具有大学文化程度，测井或石油工程专业毕业，在采油作业中有3年以上工作经验，熟悉油田注水技术。善于技术研究开发，责任心强，团队意识强，敬业、进取精神突出。安全监督（员）:具有大专以上文化程度，测井或相近专业毕业，具有基层2年以上工作经验，具有很强的安全意识和高度的工作责任心，并能起到安全监督的作用。熟悉本队的工艺流程、操作规程及相应要求。业务技术水平较高，有良好工作成绩，群众拥护，身体健康，能配合经理团结全队员工完成各项生产任务。井口操作员:具有大专以上文化程度，测井专业或相近专业毕业，具有基层2年以上工作
40、经验。熟悉生产测井的工艺流程、操作规程及相应要求。并对本队各岗位进行现场技术指导，解决生产测井工作中的技术问题，业务技术水平较高。有良好工作成绩，身体健康，能配合组长完成各项生产任务。绞车工：具有高中以上文化程度，具有很强的安全意识和高度的工作责任心，并能起到安全监督的作用。熟悉本队的工艺流程、操作规程及相应要求。业务技术水平较高，有良好工作成绩，群众拥护，身体健康，能配合组长团结全队员工完成各项生产任务。（2）岗位职责测调中队职责：1、建立健全中队QHSE管理制度和业务管理制度。2、开展全员QHSE教育活动，确保QHSE管理体系得到切实贯彻执行。2、做好生产管理、安全管理、设备管理、现场管理

44、全。3、执行油田、本公司QHSE管理体系和国家有关质量、健康、安全与环境法规、政策和规定。4、协助班长抓好负责测调作业现场的生产管理、安全管理、设备管理、现场管理工作。5、负责本班组设备仪器的维护保养工作。5、负责测调资料录取、整理、存档工作。4 风险识别与控制4.1 危险伤害的定义建立风险识别与控制体系的目的是为了准确辨识危险源、评价其风险程度，进行危险分级，并实施有效控制。要控制风险，首先要辨识危险源，评价其带来危害的严重程度和可能性，判定其风险级别，制定风险控制的措施及降低风险的优先顺序。可按以下活动辨识危险源：生产测井的地理位置；以上生产过程、活动及服务的阶段；计划或被动性的工作；确定的任务；不经常发生的任务；危险源辨识的要求：辨识应全面系统、多角度、不漏项，重点应放在能量的主体，危险物质及其控制影响因素上。辨识时应考虑以下原因：常规的活动和非常规的活动；所有进入工作场所的人员；生产作业设施；设备、检测设备；危险源的辨识方法有：询问交流、现场观察、查阅有关记录、获取外部信息、工作任务分析、安全检查表法、作业条件危险性评价法、事故树法、故障树法等等。一般采用作业条件危险性评价即LEC 评价法。测井中队危害辨识及风险评价表序号活动产品服务危害因素时态状态可导致事故风险评价风险级别现有控制措施可能性 L频繁
