Before the meeting, the way we take surveys solicited a views, combed, related to team building盖板涵深基坑开挖施工方案一、概述1.1、工程概况K42+980.6处2-4*3钢筋砼盖板涵，全部工程量为长64.35m，涵洞净
31、高4-5米，涵顶净宽10.2米，半幅盖板由(56*1.0m+2*0.75m)+1块梯形盖板组成，整体式基础；涵洞每隔4-6m设置一道沉降缝，缝内填以沥青麻絮或不透水材料；涵洞地基承载力应不小于180KPa，不足时可采用碎石换填；进出口为八字翼墙。根据公司相关规定开挖深度大于5米的为深基坑。1.2、工程地质、水文情况施工地点为沟渠，基底经长期被水浸泡，淤泥较多，其下为粉质粘土。根据我单位分析，结合现场实际情况我单位计划基坑采取按照规范放坡的方法进行施工。二、施工部署2.1、计划工期计划开完工日期为：2012年12月1日2012年12月30日2.2、人员组织机构为保证盖板涵深基坑施工质量和进度，基
32、坑开挖时配备5人进行施工，整体施工时增加至20人，能够满足施工需要。并且操作工人已经进行了技术、安全交底培训。2.3、机械设备配置挖掘机等施工机械均已到场，并检查机械的性能、状态，符合施工要求，已经对操作人员进行了培训、交底。2.4、现场准备进行基坑开挖前，水渠上下游进行围堰，必要时采用袋装土以防止渗漏，采用水泵及时进行抽水。抽水抽干后，进行淤顶高程测量，清淤采用挖掘机进行，加以人工配合，把淤泥彻底清理干净，及时进行验收，满足要求。2.5、技术准备施工前组织有关人员熟悉施工图纸及场地的地下土质，做到心中有数。根据基础平面图，定出基坑开挖线，测定高程水准点。三、施工方法及施工要点根据本工程现场的
33、地质及水文条件，依据公路桥涵施工技术规范的规定基坑采用放坡开挖施工。设置0.3米宽排水沟，排水沟设置一定纵向坡度，将水汇集至坑角集水坑内，用水泵排除至基坑外。粉质粘土按111.5放坡。3.1、基坑放样由测量人员用全站仪精确测量基槽平面位置，根据基坑底面尺寸及埋置深度、地质水文条件等确定基坑开挖的尺寸。基坑底平面尺寸比结构物基础设计尺寸各边加宽1.5米。由测量人员定出开挖边桩，连接边桩即为基坑开挖边线。在放样过程中适当加大基坑开口尺寸，以保证在基坑开挖过程中遇到不稳定土层时，能够适当加大不稳定土层的坡率。3.2、基坑开挖严格控制平面尺寸和标高，严禁扰动基底。土质基坑，基坑敞坡放坡开挖，坡率111
34、.5，采用挖掘机械开挖，人工配合，机械开挖至离设计标高约20cm时，做地基承载力试验。挖出的土、机械设备严禁堆放在基坑周边，以保证基坑边坡稳定性以及现场整洁。深基坑施工时采用“横挖法”，以基坑整个横断面的宽度和分层深度，从一端逐渐向前开挖。开挖过程中，随时注意基坑四周的土质情况。当基坑沿顶面裂缝、坑壁松塌等影响基坑稳定时，立即加固支护。基坑四周用土堆成30cm高土坝，防止水流入基坑，冲刷坑壁，基坑底设置集水坑。基坑开挖成型后，周围外设置围栏，并用密目网封闭。同时在基坑周边以及施工临时便道设置警示标志。四、工程质量保证措施4.1、质量目标：基坑施工质量合格率为100%。4.2、质量保证措施1、基
35、坑挖至设计标高后不得长时间暴露、扰动或浸泡。2、开挖过程禁止扰动基底，挖至接近基底标高时预留20cm由人工进行清理。3、基坑顶面四周设置土坝或截水沟，防止地面水流入基坑内。基底不得受水浸泡，其上淤泥或杂物必须清理干净。4、基坑开挖后，对天然基底进行检验才能进行基础施工。4.3、质量检验评定标准及检验方法基坑开挖项次检 查 项 目规定值或允许偏差检查方法和频率1平面周线位置（mm）不小于图纸要求用经纬仪测量纵横各2点2基底标高(mm)土质50水准仪：测量58点石质+50，-2003基坑尺寸(mm)不小于图纸要求用尺量4.4、施工过程控制强化质量意识的教育，组织施工人员学习施工设计图纸、质量标准及
36、验收规范。坚持岗前培训及持证上岗制度，坚持“三检、四按、五不准、六做到”，即：三检：“自检、互检、交接检”；四按：“按图纸、按规范、按工艺、按标准”；五不准：“资料不全不准开工、材料不合格不准进场、测量闭合不符合要求不准使用、上道工序不合格不准进行下道工序、达不到质量标准不准交工验收”；六做到：“方案做到合理、技术资料做到齐全、质量检验做到可靠、施工试验做到真实、测量数据做到准确、施工方法做到正确”。五、安全生产保证措施5.1、安全生产目标安全生产目标为：安全无事故。5.2、安全生产保证体系为保证工程顺利实施，项目经理部成立安全生产领导小组，全面负责本工程的安全生产，确保安全措施的落实。5.3
37、、安全管理措施1、开工前准备工作到位（1）研究本工程地质勘查报告，并掌握第一手资料。编制施工方案，有预知性地制定安全防护措施。（2）配备足够的安全防护品，口哨、安全帽、红绿旗、警示牌、红色警示灯、铁丝网。（3）开工前检查挖掘机等机械是否处于良好状态、各项制动是否有效，电缆线有无裸漏情况。2、新工人进场时，三级安全教育到位：1）由项目经理主持，对新进场工人进行对本工种具有针对性的三级安全教育，让每个工人掌握本工种的操作规程及安全注意事项，使工人明确“生产必须安全，安全促进生产”的基本原则。2）发安全知识测试考卷进行考核，确认安全教育效果，另外对特种作业人员进行特别培训，要求全部持证上岗。3、班前
38、安全技术交底到位：1）除了班组自行组织的班前安全活动外，负责该工种的工长（施工员）还用书面形式对班组进行具有针对性的安全技术交底，并履行签字手续。2）项目专职安全员总结教育和交底内容，再次详细讲解给班组的每个组员，要求做到人人会操作，人人都必须遵守安全纪律、国家有关政策法规和项目部安全管理制度。4、施工过程中的监督、检查、再教育、考核工作到位：1）在施工过程中，安全员、工长起到监督作用，发现那里出现事故苗子，及时组织人员消除，并强调杜绝类似事件不再发生。2）在施工期间，除要求施工班组例行每天的班前自检外，安全员还进行不定期复查作业情况，发现事故隐患立即勒令班组进行定人、定期限、定措施整改，并落
39、实其整改内容。3）对有些在施工过程中，安全意识淡化的工人进行再教育。4）在施工过程中对作业小组进行每天一考核，并实施奖罚制度， 5、班后总结和改进工作到位：1）通过每周的项目部安全例会对整体所做工作进行总结，吸取经验，明确责任目标，将事故隐患消除于无形。2）对工作中的不足之处进行改进，并采取有效措施进行补救，加大预防力度。3）集思广益，接纳各方面具有建设性的建议，并采取实际行动。5.4、安全措施5.4.1、机械安全1、机械作业时，施工人员严禁在回转半径内工作，尤其是土方配合人员。施工机械有专人进行统一指挥。2、夜间施工要有足够的照度，进出口设专人指挥，避免发生事故。4、人工挖掘作业人员横向间距
40、不小于2m，纵向间距不小于3m。5.4.2、安全用电措施1、电工必须持证上岗，严禁非电工人员接电，拉线。2、施工用电施工用电安装漏电保护装置。3、吊车由专人指挥，吊车臂下禁止站人，钻机、吊车与高压线保持安全操作距离。4、施工用电必须架空。5、水泵必须接地，单机单闸。5.4.3、施工措施1、开挖边坡土方,严禁切割坡脚,以防导致边坡失稳。2、基坑开挖时要对附近地下缆线采取保护措施。标明位置，现场设专人看护指挥。3、开挖过程中注意土质变化情况，土质不好时采取支护措施。4、基坑四周做1.2米高的临时围栏，并用密目网封闭，夜间设红色警示标志。5、基坑顶1米以内不得堆土堆料。6、施工时将基坑边上散土和活动
41、碎石清理干净。7、基坑开挖前完善四周临时排水设施，对现况排水沟渠进行疏通，防止水冲基坑。8、加强施工现场边坡监控，在基坑周围共布置测点，并在周围布设沉降观测点；结构施工期间做到每日一测，确保在第一时间发现安全隐患。9、 建立完善的预警报告制度，在监测过程中发现坡顶连续三天以上位移速率大于3mm且不收敛、坡顶累计位移50mm、坡顶地面明显沉降、地面明显开裂及时上报。5.5、防雨措施1、基槽开挖前准备好防洪设备器材，开挖边坡比常规放缓，坡面根据情况加以覆盖。2、基槽挖好后及时做好协调工作，尽量减少土基暴露时间。如结构当天不做，槽底要留20厘米土待第二天做结构前再清到槽底。3、基础开挖施工时，在基坑
42、周围设宽30cm，高30cm的挡水埂，防止地面水灌入基坑，基坑内设集水井，配足抽水机抽水。基坑挖好后及时浇筑混凝土或垫层，防止被水浸泡。4、随时注意天气情况，下雨前在坑顶设置土埂，并备足塑料布，在下雨前覆盖在坑壁四周，防止雨水冲刷坑壁。5.6、安全事故应急预案5.6.1、应急救援组织机构组 长：卞开春副组长：周从育、左勇组 员：尤亮、张干、张旭、潘思扬、梁浩及施工队负责人组织机构下设：报案组、急救组、维护秩序组、事故调查组。5.6.2、应急救援机构职责1、组长职责：全面负责处置突发性安全事故工作，及时召集领导小组成员研究处置方案，布置任务，下达命令，启动应急救援预案。2、报案组职责：发生突发性
43、安全生产事故，现场负责人应根据事故情况立即向有关领导汇报，并拨打相应急救中心电话，告知事故种类、事态、地点、联系人等，同时紧急通知急救组、机械设备组和维护秩序组，启动应急救援预案。协助事故的调查与处理工作。3、急救组职责：接到报案组通知后，立即赶赴现场，根据现场事故种类，立即组织抢救，协助事故的调查与处理工作。5.6.3、危险因素分析及对策基坑主要危险因素为：落物伤人、塌方、人或者机械产生的安全事故。原因及对策如下：塌方：原因：主要是因为土质不好，放坡不够，或者有地下水及雨水冲刷。对策：根据地勘资料确定合适的边坡，并且在开口时适当加大尺寸，当遇到不稳定地质条件时适当加大坡率，通过该土层后正常放
44、坡。对于开挖过程中出现的塌方根据不同情况采取锚固支撑、斜柱支撑等措施，防止边坡塌方，具体施工方法见下图：施工期间加强滑坡的监测监控，对点位有变化需立即停止现场的施工，加密观测次数，仔细分析点位的变动原因，确定具体处理方法施工前必须做好截水沟和排水沟，截断山体水流。落物：原因：坑边坡上松动石块没有清理干净或者施工过程中落物。对策：将坑边异物清理干净。5.6.4、事故应急措施1、发生坍塌事故后，立即报告应急抢险指挥小组，由项目经理负责现场总指挥。发现事故人员通知现场安全员，并由安全员组织施工人员紧急撤离至安全区域。2、如有人员受伤，立即拨打“120”急救中心电话取得联系，详细说明事故地点、严重程度
45、，并派人到路口接应。3、在向有关部门电话求救的同时，对受伤人员在现场安全地带采取可行的应急措施，如现场包扎止血等措施。防止受伤人员流血过多造成死亡事故发生。4、立即上报公司及有关部门，并启动公司应急救援预案。5、如有人员被掩埋，要采取有效安全防护措施后，组织人员按部位进行人员抢救，尽快解除重物压迫，减少伤员挤压综合症的发生，并将其转移至安全地方，防止事故发展扩大。六、文明施工保证措施6.1、文明施工目标文明施工目标：创文明工地达标工地。6.2、文明施工措施1、施工现场设排水设施，保持场内无积水；施工临时道路保持平整、坚实、畅通。2、现场不准随地大、小便，现场内的土方、零散碎料和垃圾及时清理、码
